How to make a complaint

You can make a workplace complaint online, by fax, by post or in person.

The online and fax/postal versions of the form are the same.

Lodge online

 Lodge your workplace complaint online

Note: First-time users will need to register and create an account.

  • Allow about 20 minutes to register and complete the form.
  • There's no timeout period. You can save the form to your computer, then complete and submit it later.
  • Questions marked by a red asterisk are mandatory.
  • Give as much detail as you can in the form to help us assess your workplace complaint.
  • You can attach up to 5 electronic copies of supporting documents. If you have more, tell your assigned Fair Work Inspector when they contact you.
     

Or, you can send the extra supporting documents by post. Write your name and date of birth on all documents so we can match them to your workplace complaint and mail to:

Check the status of your complaint:

Lodge by fax

  • Write your name and date of birth on any supporting documents you include.
  • Fax your completed form, together with your supporting documents to 02 6250 4480.

Lodge by post

  • Send copies of supporting documents, not originals. Write your name and date of birth on any copies of supporting documents you include.
  • Post your completed form, together with the copies of your supporting documents to:

    Adelaide SA 5001

Make sure your workplace complaint form is completed and signed before sending it to us.

We may return unsigned forms, as investigations can't begin without the necessary information and your permission.
